consumer reporting agency in connection with your application for joining Commercial Cleaning
Services as an independent contractor. Any information contained in these reports may be taken into
consideration in evaluating your application.
For Commercial Cleaning Services purposes, a consumer report consists of a credit check, criminal
history records, motor vehicle records, education and independent contractor employment history.
The Federal Fair Credit Reporting Act is designed to promote accuracy, fairness and privacy of
information in the files of every CRA. Most CRAs are credit bureaus that gather and sell information
about you to creditors, employers, landlords and other businesses. As part of the FCRA, a CRA may not
provide information about you to a prospective Company without your written consent.
If it appears likely an adverse decision regarding your association with Commercial Cleaning Services
may be made based entirely or in part on information contained in these reports, you may be notified
of this possibility before a final decision is made. At that time, you will be provided with a copy of any
consumer report(s) Commercial Cleaning Services has obtained on you, as well as a summary of your
rights under the FCRA.
Your consent is required by law for the release of this information. Your signature on this form
indicates you have carefully read and understand this notice and authorizes the release of a consumer
report to Commercial Cleaning Services for the purposes described above.
